How to Download & Open in Acrobat Reader
Your Patient SOAP Note is being sent to you as an interactive pdf form file to the email address you provided when you ordered. The download will remain active for one week.
Your Patient SOAP Note is being sent to you as an interactive pdf form file to the email address you provided when you ordered. The download will remain active for one week.
- Open on the mobile device where you want the file to appear.
- Tap on the Download button when prompted.
- When the file has finished downloading, it will appear on your screen. Tap "Open In" and choose Acrobat Reader (If you don't have Acrobat Reader, download it free from the App Store).
- When open, tap the page once to access the Acrobat Reader controls; the tool bars will remain for a few seconds and close automatically if not in use; you can also tap the screen once to make them go away (be careful not to tap on a link or too close to either side. Tap the eye icon and select "Single Page" from the menu. Tap "Done."
- Use the "Duplicate" tool in Acrobat Reader to make at least one copy: Go to Document > Edit > and choose the Duplicate tool from the bottom of the screen. Make more duplicates if you need to treat more than one patient before opening a working file. You can rename a file at any time by tapping "Edit" in the document section then tapping the text icon. Tap "Done" when you are finished renaming the file. It’s a good idea to at the patient’s name to the file name before opening and using a Patient SOAP note.
- Play with using the Acrobat tools. Note that any marks or comments generated by the tools will need to be cleared one at a time by tapping the mark or comment and then tapping "Clear."
How to Navigate & Fill Out a Patient SOAP Note Form
- Tap a hyperlink in the Main Menu to jump to that page; tap the Main Menu text to jump back. You may also navigate the form by using the Bookmarks. To use bookmarks, tap the page once to access the Adobe Reader controls, then tap the bookmark icon (an open book) at the bottom right corner of the page. A bookmark menu will appear. Tap a link to go to that page.
- You may also move forward and backward by swiping a page left or right, or by tapping once on the left or right side of a page.
- There are three types of fields: Text Fields, Radio Buttons, and Check Boxes.
- Single-line Text fields are used for names, phone numbers, email addresses, etc. Large Text fields have multi-lines and scrolling. In these windows the text will automatically wrap around to the next line. Tap "Return" on your keyboard to start a new paragraph. Try to align the "Time" fields with the text on the larger field next to them on the pages with Symptoms, Physical Exam, Current Problem List, Anticipated Problem List, and Field Treatment Plan. Tap a field to begin typing. When done, move forward to the next field by tapping "Next"; move back to the previous field by tapping "Previous"; clear the field by tapping "Reset"; or tap "Done" to close the window. You may reopen the field to add, delete, or change the text at any time by tapping the field.
- Radio Buttons (tap to choose; you may only choose one; tap again to unselect)
- Check Boxes (tap to choose; you should choose all that apply; tap again to unselect)
How to Email a Completed Patient SOAP Note Form
- Your phone or tablet will automatically save the document; however, to make sure you don't lose it, cc a copy to yourself each time you send it to someone (physician, program director, search & rescue team leader, etc. Remember to add the patient’s name to the file name.
- To Email the file to a physician, program director, search & rescue team leader, etc. Tap the page once to expose the Acrobat Reader tools. Choose email tool (second tool from the right at the top of the page) and tap "E-mail Document" and choose Share Flattened Copy. Before sending your email, consider taking and attaching photos of the patient, site, etc. You can also send photos in a separate email. It is your responsibility to protect a patient's privacy; share only the information necessary for their assessment, treatment, and evacuation.
- To clear the form: Go to the Main Menu and tap on the "Clear Form" button located on the upper right hand corner of the page. Be patient, clearing an entire form takes a few minutes. Remember that if you used any of the tools in Acrobat Reader, they will need to be cleared one at a time by tapping the mark or comment and then tapping "Clear."
