WMTC Patient SOAP Note ~ This is the same 8.5 x 14 inch SOAP Notes used in our WFA, WAFA, WFR, & WEMT courses. You can download and print them for your personal or program use; however, you are prohibited by copyright law from printing them for resale or teaching. Copies of these notes may be purchased from our webstore printed on standard white paper in tablet form (20 notes per tablet) or on weather-proof, tear-resistant synthetic paper; please contact the WMTC office for details.

WMTC Patient Note ~ These 8.5 x 11 inch Patient Notes were developed for our Wilderness Basic Life Support (WBLS) course. You can download and print them on plain paper for your personal or program use; however, you are prohibited by copyright law from printing them for resale or teaching. Printed copies of these Patient Notes are not available for purchase at this time.

WMTC Trip Drug Log ~ All trips should have drug log in their first aid kits to keep track of both OTC and Rx drugs. You can download and print them on plain paper or on weather-proof paper for your personal or program use; however, you are prohibited by copyright law from printing them for resale.
